Students entering school must get the required doses listed in this chart to be in compliance with immunization requirements. The requirements follow the national Advisory Committee on Immunization Practices (ACIP) Immunization Schedule with requirements specified by grade level.
We want to let you know about some changes to stage immunization rules that will take effect on August 1, 2020.
Starting in the 2020-2021 School year, all immunizations records turned into schools must be medically verified. This means they must be on a Washington Certificate of Immunization Status Form (CIS) from a health care provider. Or, parents must attache medically verified proof of immunizations to the handwritten CIS showing their child's records are accurate.
Please Note: Immunization records must be turned into the school on or before the first day of attendance. This includes when a student moves within the district. Students will not be allowed to attend school until the records are provided.
A Washington Certificate of Immunization Status Form (CIS) form signed by a health care provider.
A CIS filled out by a parent/guardian with attached medical records.
A CIS printed by a health care provider or school from the Washington State Immunization System.
A CIS printed from MyIR, a free state Department of HEalth online tool that allows families to view and print their official immunizations records themselves. Go to https://wa.myir.net/login/ to sign up.
Parents/guardians requesting an immunization exemption for their child must complete a Certificate of Exemption (COE).
If your child meets the new immunization requirements and has current records on file, you don't need to do anything.
Starting on August 1, 2020, the requirement for a Tdap dose changes from grade 6-12 to grade 7-12. All students entering 7th through 12th grades must have one booster dose of the Tdap vaccine. In the 2020-2021 school year, all students in 7th grade must have received their Tdap dose on or after age 10 years. Students in 8th through 12th grades must have received their Tdap dose on or after age 7 years.
